- Pernell Roberts as Dr. John "Trapper John" McIntyre, Chief of Surgery and former member of the 4077th Mobile Army Surgical
Hospital during the Korean War.
Jessica Walter appeared as Melanie McIntyre, Trapper's ex-wife.
- Gregory Harrison as Dr. George Alonzo "Gonzo" Gates, a brash, rebellious
surgeon who served three years
with a Mobile Army Surgical Hospital during the Vietnam War. Gonzo lives in a
RV (dubbed the "Titanic") located in the hospital parking lot.
When he had a stroke, he ended his
career as a surgeon and left the hospital with his new
wife, Fran Brennan (Andrea Marcovicci).
